Enterprise Hosting ACH Authorization

Thank you for choosing Enterprise Hosting. The yearly cost of this hosting plan is two hundred and eighty-six dollars ($286.00), billed annually. By filling out and submitting this ACH Agreement form, you acknowledge that you have read, understand, and agree with our terms of service. You are granting Palm Beach Cuisine, Inc. permission to debit the bank account belonging to the business named below for the cost of services you choose here. This plan provides your business with our standard Managed Hosting, Website Security, Site Monitoring & Remediation, Backups, and 25 Email Boxes. Options such as a dedicated IP address, additional email boxes, and Content Management Tools ( SitePoster ) are available if needed.

Palm Beach Cuisine, Inc. does provide automatic renewal of provided services. You can opt out of services at any time by phone or email. We have a no-refund policy, and we do not prorate our services. All purchases are final. If you purchase a hosting plan today and decide in three months that you wish to cancel, you will not receive a reimbursement of funds.

Upon submitting this form, you will be notified by one of our representatives so that the necessary site migration arrangements can be made. Only after site migration has been completed successfully will your account be debited for services rendered.

If you are requesting development services besides website hosting and received a quote for a website build, please include the amount quoted in the Development Services field. A fifty (50%) deposit is required for all development services before work begins, and your business bank account will be debited for the amount equal to fifty percent (50%) of the agreed-upon quote. When your website is completed and live, the remaining development services and hosting balance will be charged.
